The design is discussed very shortly in the book ‘Japanese Erotic Art, the Hidden World of Shunga‘ (Fig.52 on p.263). According to the author the three phallus figures are toys.
In my opinion it could very well be a fantasy scene since there seems to be an interaction between the hand and the three penises.
